Who We Serve:
Adults on Medical Assistance (MA) who:
- Are experiencing housing instability or homelessness
- Have a disabling condition (mental health, substance use, physical, developmental)
- Need support to find or maintain housing
- Are transitioning from institutions or treatment facilities
Now Accepting Referrals From:
- County & Tribal Case Managers
- Healthcare Providers (Hospitals, Clinics, Mental Health Centers)
- Disability Service Organizations
- Homeless Shelters & Transitional Housing Programs
- Social Service Agencies
- Individuals & Families

- DHS-7307: Person-Centered Plan
- Begin services upon DHS authorization
